Preparing for Your Transplant (Pre-Op)

Thorough preparation minimizes surgical risks and maximizes the chance of immediate graft function. While you wait for your transplant date, strict medical optimization is mandatory.

  • Rigorous Medical Clearances: Comprehensive cardiac stress tests, pulmonary function tests, and resolving any active infections (including dental cavities) are required to ensure you can safely undergo anesthesia.
  • Dietary Conditioning: Patients on dialysis must strictly adhere to low-potassium, low-phosphorus, and fluid-restricted diets to keep their body in the best possible physiological state prior to surgery.
  • Immunological Readiness: Final blood draws are taken to verify crossmatching and ensure no new donor-specific antibodies have developed.

The Critical Phase: The First 90 Days

The first three months following a transplant are the most sensitive. Your immune system is actively adapting to the new organ, requiring extreme vigilance.

  • Immunosuppression (Anti-Rejection Meds): You will begin a strict, lifelong regimen of medications (such as Tacrolimus or Mycophenolate) to suppress your immune system and prevent it from attacking the new kidney. Timing and dosage accuracy are non-negotiable.
  • Infection Prevention: Because your immune system is suppressed, you are highly susceptible to infections. Strict hygiene, avoiding crowds, and safe food handling are mandatory.
  • Vitals Monitoring: Patients are taught to meticulously log their daily blood pressure, temperature, body weight, and urine output. Rapid changes can be early indicators of complications.

Recognizing Organ Rejection

Rejection does not mean the kidney is lost; it simply means your immune system has noticed the foreign tissue and requires a medication adjustment. Contact our clinic immediately if you experience:

  • A sudden decrease in urine output or sudden fluid retention (swelling in legs/ankles).
  • A fever of 100°F (37.8°C) or higher.
  • Pain, tenderness, or swelling directly over the transplanted kidney site (lower abdomen).
  • A sudden, unexplained spike in your blood pressure or serum creatinine levels.
